Heartland Health Center

3C was retained by the Heartland International Health Centers to deliver their newest primary care center in Chicago. The turnkey development is the first LEED Platinum Community Health Center in the State of Illinois, providing access to affordable primary care to the medically underserved and uninsured in Chicago’s Rogers Park, Edgewater and West Ridge neighborhoods.

3C recognizes that today’s urban communities face many challenges in delivering and providing their residents with access to high-quality, affordable healthcare. Together with community stakeholders, we created a development plan, assembled an expert project team, and retained financing while acquiring 43,400 square feet of non-contiguous real property in less than 120 days.

Together with the design/build team of MDT Architects and Madison Construction, 3C has significant experience with Federally Qualified Health Centers on a national level. Heartland Health Center – Devon will set a new standard for innovation in healthcare design and construction. The $8.5 million facility will encompass 9,000 square feet of primary care exam rooms, administration and office space, as well as a community room that will accommodate 100 people.

By leveraging grant-funding sources, our team was able to incorporate the most advanced, energy efficient elements into the project design. Upon completion, the building will feature geothermal heat pumps, photovoltaic panels, recycled materials, and advanced lighting controls.
